No, Our services are free.
Do I need a medical history?

No! We rendered routine services. Only if the medical doctor on site would request your medical history.
What information should I bring?

State or any valid identification is fine.
How much are the appointments?

Services are free. However, travel to home, hospice, or hospital small charge to cover travel is required. if you would like to make a donation to help with cost associated with the clinic they will be accepted.
Can I mail my prayer requests?
